Before continuing to our online booking page, please read and agree to our terms and conditions below:
COVID-19 Screening questions
I affirm that I have NOT had any of the following in the past 14 days:
- A positive COVID-19 VIRAL test
- Close contact with someone diagnosed with COVID-19 (Close contact means having been within 6 feet of that person for an extended time or being exposed to their cough or sneeze)
- Traveled outside of New York State (IF YES, see this webpage for updated guidance)
- History of any respiratory conditions that affect your breathing or oxygen levels
- Cough
- Shortness of breath or difficulty breathing
- Fever
(Or at least 2 of the symptoms below):
- Repeated shaking with chills
- Headache
- New loss of taste or smell
- Muscle pain
- Sore throat
If you feel sick or have any of the aforementioned symptoms prior to your appointment, it is important that you call us ASAP so that we can determine if you need to reschedule your appointment.

Online Booking page: On the following pages you can select the service(s) you would like to book, your practitioner, and your preferred date and time range. You can also add yourself to a waiting/call back list for unavailable appointment times. If you are an existing client/patient and this is your first time booking online with us, you will need to register to create an account and provide a valid credit card to hold your appointment. The credit card you provide will only be charged if we need to enforce our cancellation/rescheduling policy. Payment for your service(s) will be collected on the day of your visit by the payment method of your choice. After registering, you will receive an email confirming your registration (this is not an appointment confirmation). You will still need to finish booking your appointment, after which you will receive a confirmation email listing your appointment details.
Please note: If your desired appointment is not available or if you want to book a same day appointment, please call us to see if we can accommodate you. If you have never had a service with us or you are trying to book your first acupuncture appointment at Slope Wellness, you must also be booked by phone.
Cancellation/rescheduling policy: No-shows and appointments canceled or rescheduled with less than 24 hours notice will be charged a cancellation fee equal to half of the service fee. Please be advised that you are not able to cancel or reschedule appointments through our online booking system. If you need to cancel or reschedule, please call or email our office.
